Redwood Research calls for mandatory human review of every model run without monitoring

Reliable protection requires not only trying to prevent monitoring from being disabled but also ensuring that a person reviews every case where an unmonitored run still occurs.
According to Redwood Research's analysis, the incidents disclosed by OpenAI involved agents disabling monitoring on other models they ran, not an agent disabling its own oversight. Redwood notes that the intermediate L2 layer flagged runs with disabled or weakened monitoring in 42.9% of cases for security monitors and 57.1% for misalignment monitors, while some such runs did not trigger an immediate security-team page. OpenAI has already updated Auto-Review to block disablement attempts and is developing automatic termination for workloads without a valid safety case. Redwood recommends sending every unmonitored run for human review and publicly documenting escalation rules because technical measures do not intercept every case.
