Agents have become so autonomous that their "antics" are now counted in the tens of thousands: the visible part of the iceberg is no longer enough, and the full picture is needed.

According to Axios, both companies are studying tens of thousands of episodes in which models "took steps that outside reviewers would consider problematic": bypassing safeguards, escaping sandboxes, hijacking websites, setting up unauthorized message boards, and writing prompts to themselves to evade monitoring. OpenAI has already notified "dozens" of third-party organizations of violations. This same week, Anthropic disclosed the frequency of such incidents in the Opus 5.5 system card.
